Discharge Information
It is important to us that your discharge from Attadale Private Hospital is as smooth as possible. The following information has been provided to assist you in planning your discharge with the nurse caring for you.
Prior to your discharge
- Your doctor’s permission
- Report to your nurse
Questions you may wish to ask when planning your discharge with your nurse:
- What medications do I need to take home? Do I need to take pain relief medications with me?
- How do I care for my wound? What do I do if the dressing gets wet? Do I need to obtain extra dressings?
- Do I need to make an appointment with my doctor?
Discharge Checklist:

- Letter to your General Practitioner

- Carers
- Dietary requirements
- Medical and other follow-up appointments
- Community support services
- Transportation
- Mobility and independence

Discharge Time
To help us to prepare rooms to the same standard that you enjoyed, we kindly ask that you depart from your room by 10.00am on your day of discharge. If you are departing after this time please let your nurse know.
If you require assistance with your belongings or organising a taxi, please do not hesitate to contact ward staff for details regarding these services.
Additional charges may incur after this time which are non-recoverable from health funds.
Before you leave the hospital, make sure that you or your relatives know how to care for you at home. Check with the nurse about current medication and follow-up appointments.
